Request for submitting a medical error complaint

We try to resolve complaints in a timely manner. However, investigation might take months, depending on the complaint's complexity and type of investigation required.

Description

Through this service, you can request an investigation in medical error complaint against healthcare services provided by any licensed healthcare facility/professional in the Emirate of Abu Dhabi.

Process

01. Register and create an account (could use UAE pass)

02. Submit the application and attach the required documents

03. Receive email notification of successful submission

04. Receive the committee report through DOH electronic complaint management system

Required Documents

Complete the online complaint form

Copy of the Emirates Identity Card for the patient and the complainant

If he/she is not the patient

Death certificate

To submit the complaint on behalf of the deceased by a first-degree relative

Legal power of attorney

In case there is other authorized person to submit the complaint.

Medical reports
