Welcome to the Abu Dhabi Healthcare Facility Survey
As part of an ongoing process to improve capacity planning for health in Abu Dhabi, Department of Health is issuing a Healthcare Facility Survey in 2018.
Facility managers and operators are required to complete an individual survey for each health facility licensed to operate in Abu Dhabi as the following types:
- 01Hospital (all types)
- 02Day Care Surgery Centre
- 03Diagnostic Centre
- 04Fertilisation Centre
- 05Dialysis Centre
- 06Dental Centre or Medical Centre
- 07General Dental Clinic, General Medical Clinic, Specialised Dental Clinic or Specialised Medical Clinic
NB: Pharmacies, Optical Centres and School Clinics are not required to complete the survey.

About the Healthcare Facility Survey
The survey had been developed by Department of Health, with support from TAHPI. This is not a performance audit but a survey that will accurately capture the latest information on all health facilities and services in Abu Dhabi. The survey will be used to better understand the capacity and future requirements of Abu Dhabi’s health system and the survey results will be made available to the public. This will be done via the recently launched Department of Health Public Mapping Tool that enables members of public to search, identify and locate their health care needs. Department of Health looks forward to the full cooperation of facility owners, managers and operators in planning the future of healthcare services in Abu Dhabi. Participation is compulsory and facility managers and operators are requested to complete the survey in its entirety by October 22nd 2018.

Question: I have not received an email notification?
Answer: Make sure you have created an account with the correct email, you will receive an email requiring you to confirm the email address provided. Reset password by clicking “Forgot password” and a new link will be sent to your email.
Question: Does this survey replace having to put an application in for the Facility Licensing requirements?
Answer: No, this survey does not in any way change your obligations in relation to the Facility Licensing requirements. Any amendments or changes to your service provision still need to be
