Description

DOH license number of the Provider, Insurer or TPA sending information. If the sending healthcare entity is not licensed by DOH, enter “@” followed by the name of the entity. For transaction pairs the receiver of the first transaction must be the sender of the second transaction, e.g., if a TPA receives a ClaimSubmission from a provider, then that TPA (not the insurer) must send the RemittanceAdvice to the provider.
