The amount a patient owes a provider (regardless of whether the provider received the payment) according to the terms of their insurance plan/product, or the entire price of treatment in the case of medical tourism, self-pay or other non-insurance sources. If the patient has no insurance coverage for the visit, they are considered self-pay and liable for the entire amount, per their signed consent for treatment.
